Loan Limit
The amount of loan to be granted will depend on the severity of the case of the patient and the paying capacity of the borrower. The maximum loanable amount is P150,000.00 with an interest rate of 10% per annum and payable for a maximum of three ( 3) years.

Initial Requirements
- Certification of net take home pay
- Certification of leave credits of at least 30 days and 60 days for those availing more than P100,00.00
- Signature of spouse
- Collateral/Submit origjnal copy of title
- Deed of conveyance/ Deed of undertaking
Additional Requirements
- Doctor's Recommedation ((elective/scheduled surgery)
- Medical Certificate ((duly signed by the attending physician)
- Official Receipts
